The short answer
- Fotona 4D is a protocol, not a machine: four steps on one Fotona laser combining Er:YAG (2940 nm) and Nd:YAG (1064 nm), starting inside the mouth and ending with a light peel.
- The FDA cleared the laser, not a "laser face lift": the cleared uses include wrinkles and resurfacing, but not Fotona 4D, lifting or tightening.
- The evidence is real but small: the best study, of 13 women, measured about half a millimetre of tightening beside the eyes and one grade softer nasolabial folds, partly fading by six months.
- Published Dubai prices in October 2026: AED 3,500 for a full-face session, AED 9,450 for three sessions and AED 5,500 for face and neck. No Abu Dhabi clinic we found publishes a price.
- It is a prescription laser, so book only at a clinic licensed by the DHA, DOH or MOHAP. Check the clinic's licence first.
Fotona 4D is heavily marketed in Dubai as a non-surgical facelift with no needles. Prices differ by a factor of ten, "5D" means something different at every clinic, and almost nobody links to a study. This guide sets out what we could verify, including for the Fitzpatrick IV to VI skin common in the UAE.
What is Fotona 4D, and which laser does it use?
Fotona 4D is a four-step facial protocol performed with one laser from the Slovenian manufacturer Fotona that holds two wavelengths: erbium (Er:YAG, 2940 nm) and neodymium (Nd:YAG, 1064 nm). Fotona writes it as Fotona4D and calls it a non-surgical face lifting procedure.
Er:YAG light is absorbed strongly by water, so it stays in the top layers of skin or mouth lining: low settings heat, higher ones remove a very thin layer. Nd:YAG light travels deeper into the dermis and is absorbed less by surface pigment, so it heats deeper tissue. Together they let one session work at several depths.
Fotona lists several platforms for the protocol: the SP Dynamis, the newer Dynamis Max, the SP Dynamis Nx Line and the TimeWalker II Fotona4D, a smaller system built for face treatments. StarWalker is Fotona's Q-switched and picosecond line for pigment and tattoos, a different tool. Lighter protocols exist: one Dubai price list in October 2026 offered 2D, 3D and 4D face sessions at AED 2,100, AED 2,750 and AED 3,500.
What are the four steps, and what do 5D, LipLase and SmoothEye add?
The steps run from inside the mouth outwards: SmoothLiftin, FRAC3, PIANO and SupErficial, each using one wavelength in one mode.
| Step | Laser and mode | Where | What it is meant to do |
|---|---|---|---|
| 1. SmoothLiftin | Er:YAG, non-ablative SMOOTH mode | Inside the mouth, on the cheek and lip lining | Tighten around the mouth and soften nasolabial folds from the inside |
| 2. FRAC3 | Nd:YAG, short pulses in a fractional pattern | Facial skin | Target deeper pigment and texture flaws while leaving the surface intact |
| 3. PIANO | Nd:YAG, very long pulses swept across the skin | Face, often the neck | Heat the deep dermis evenly for overall firming |
| 4. SupErficial | Er:YAG, light ablative mode | Skin surface | A light laser peel for tone and texture |
The inside-the-mouth step sets Fotona 4D apart and has the most published research. Deeper erbium treatment for scars or sun damage is a more aggressive use of the same wavelength, covered in our guide to laser skin resurfacing in Dubai.
What is Fotona 5D?
5D is a clinic term, not a Fotona protocol: we found none on Fotona's website. Clinics use it for the four steps plus one extra pass, defined variously as an additional SMOOTH-mode tightening pass, an extra Er:YAG resurfacing pass, or extending treatment to the neck and hairline. No Dubai clinic we checked published a 5D price, so ask what the fifth step is and how the price compares with 4D.
LipLase and SmoothEye
- LipLase uses the Er:YAG SMOOTH mode on the lips, sometimes with Nd:YAG PIANO heating, sold as lip plumping without filler. Expect subtle fullness and softer lip lines, not a filler result. Dubai prices start from AED 490 a session when you book three.
- SmoothEye applies the SMOOTH mode above, below and beside the eyes for fine lines and crepey eyelid skin. One Dubai price list shows AED 1,500 a session or AED 4,050 for three.
Is Fotona 4D FDA approved?
No treatment called Fotona 4D has been cleared by the US FDA as a face lift. What the FDA has cleared is the laser: Fotona's Dynamis Pro family, which includes the SP Dynamis, most recently on 19 September 2026 (510(k) K252723).
A 510(k) clearance means the FDA agreed the device is substantially equivalent to lasers already on sale for the listed uses. It is not an approval or a verdict on a protocol. The cleared prescription uses include:
- Er:YAG: soft tissue resurfacing, skin lesions, and "intra-oral soft tissue incision, excision, ablation, coagulation", which is surgical wording, not a lifting claim.
- Nd:YAG: wrinkles, mild to moderate inflammatory acne, vascular and pigmented lesions, and hair reduction on Fitzpatrick I to VI skin.
Skin tightening, lifting, nasolabial folds and the name Fotona4D do not appear. So "uses an FDA-cleared laser" is accurate, while "FDA-approved facelift" is not. Fotona states that all its medical lasers are CE marked in Europe, and in Dubai the DHA requires every cosmetic laser to be registered and approved under the UAE's medical device rules.
Does Fotona 4D really work? What the studies show
It produces measurable change in small studies, mainly softer nasolabial folds, finer pores and slight tightening beside the eyes, but the effects are modest and no study compares it with HIFU, Morpheus8 or a facelift.
Much of what clinics quote comes from the Journal of the Laser and Health Academy, published by the Ljubljana academy that trains Fotona users; Fotona says its content is not official product information. The table uses only independent journals.
| Study | Who and what | What it found |
|---|---|---|
| Qi et al., Lasers Surg Med, 2026 (Beijing) | 13 women, four Fotona 4D sessions a month apart, measured with 3D imaging | About 0.5 to 0.6 mm of measured tightening beside the outer eye at three months, partly fading by six; a slight rise at the cheekbone; nasolabial folds one grade better; finer pores |
| Moftah et al., Skin Res Technol, 2020 (Cairo) | 20 women, five monthly SMOOTH sessions, inside the mouth on one side and on the skin on the other | Thicker dermis on scans on the inside-the-mouth side, but no visible difference; patients preferred the skin side |
| Ebrahim and Gharib, J Cosmet Laser Ther, 2018 (Egypt) | 20 patients, inside-the-mouth step only, six sessions | Nasolabial folds improved at six months on blinded scoring; no control group |
| Badawi et al., Lasers Med Sci, 2022 | 30 patients, SmoothEye-type treatment, three sessions | Lines around the eyes improved and held at 12 months; one author is affiliated with Fotona |
| Mohamadi et al., Lasers Med Sci, 2025 (Tehran) | 36 women, Fitzpatrick III to IV, combined Nd:YAG and Er:YAG laser vs an Er:glass laser | Both groups improved; only the combined laser helped lines at the corners of the mouth |
Taken together, the inside-the-mouth step changes the tissue, the full protocol's measurable tightening is a fraction of a millimetre, and the effect partly fades within six months. None of the studies reported separate results for Fitzpatrick V or VI skin. If your folds come from lost volume, our guide to getting rid of nasolabial folds in Dubai explains why filler or a biostimulator often does more.
How many sessions, is it painful, and what is the downtime?
Plan on three to five sessions about a month apart, heat rather than pain, and a day or two of redness, with a few days of light peeling if the peel step is done.
Sessions. Dubai clinics typically quote three to five sessions three to six weeks apart, then maintenance every six to twelve months. Clinics book 45 to 90 minutes; Fotona gives 30 to 45 minutes of treatment.
Pain. Fotona markets the treatment as needing no needles or anaesthesia, and one study called the inside-the-mouth step painless. The outer steps feel hot, especially the long PIANO passes, and some clinics apply numbing cream first.
Downtime. The Beijing study recorded only temporary redness and swelling; in the eye study, patients had mild swelling and redness for one to two days and superficial peeling for four to six days. Most people work the next day; clinics advise avoiding direct sun and pausing retinoids and acids for about a week.
Results. Fotona says the full effect builds over two to three months, improving for up to six. Judge the course three months after the last session, and start three to four months before a wedding or Eid.
How much does Fotona 4D cost in Dubai and Abu Dhabi?
In October 2026, published Dubai prices were AED 3,500 for a full-face session, AED 9,450 for three sessions and AED 5,500 for face and neck, with small-area offers from about AED 1,000. Estimates on three other clinic pages run from AED 1,500 to AED 9,999 a session, because clinics price by area, by number of steps or with add-ons.
| What you are paying for | Where it was published | Price (October 2026) |
|---|---|---|
| Full face, single session | Dubai clinic price list | AED 3,500 |
| Full face, course of three | Same clinic | AED 9,450 |
| Face and neck, single session or three | Same clinic | AED 5,500 or AED 14,850 |
| One-hour Fotona 4D session | Jumeirah provider on a booking platform | AED 3,500 |
| Fotona 4D, depending on the plan | Clinic group with Dubai and Abu Dhabi branches | AED 1,490 to 2,400 |
| Small to medium area, advertised offer | Two Dubai clinics | AED 999 to 1,000 |
| Full course estimate, three to four sessions | One Dubai clinic | AED 8,000 to 18,000 |
| SmoothEye, single session or three | Dubai clinic price list | AED 1,500 or AED 4,050 |
| Fotona lip treatment (LipLase type) | Three Dubai clinics | AED 490 (booking three) to 1,499 |
| Fotona 5D | No published price found | Quoted after consultation |
Abu Dhabi. No Abu Dhabi clinic we found publishes a Fotona 4D price; clinics in the capital and Al Ain quote after consultation, and the group with branches in both emirates does not say whether its Yas Island branch matches its range.
The real cost of a course. Three full-face sessions cost AED 9,450 to 10,500 at published Dubai prices, or AED 14,850 for face and neck. Add one maintenance session at AED 3,500 and a first year on the full face comes to about AED 13,000. When comparing quotes, confirm per session or per course, the area covered, whether all four steps are included and whether 5% VAT is added, since one low advertised price excluded VAT.
Fotona 4D vs HIFU, Ultherapy, Morpheus8, thread lifts, Endolift and fillers
Fotona 4D sits at the gentle end of the lifting options: it suits early laxity, fine lines and dull texture, while sagging jowls, deep folds and lost volume usually respond better to something else. Prices match our dedicated guides.
| Option | How it works | Best for | Downtime | Typical Dubai price |
|---|---|---|---|---|
| Fotona 4D | Two-wavelength laser heat, inside the mouth and on the skin | Early laxity, fine lines, texture | A day or two of redness | AED 3,500 a session; AED 9,450 for three |
| HIFU (for example Ultraformer) | Focused ultrasound down to the deeper SMAS layer | Mild to moderate jowl and jawline laxity | Minimal | AED 4,000 to 5,000, full face |
| Ultherapy | Focused ultrasound with live imaging | The same concerns as HIFU | Minimal | AED 12,000 to 14,000, face and neck |
| Morpheus8 | Radiofrequency microneedling | Laxity with scars, pores and texture | A few days of redness and swelling | AED 1,800 to 3,000 a face session |
| Thread lift | Dissolvable threads lift tissue mechanically | Visible sagging in the cheeks and jowls | 3 to 7 days of swelling | AED 4,000 to 9,000 |
| Endolift | A thin laser fibre placed under the skin | Lower face and neck laxity | Some swelling for a few days | From about AED 3,000 per small area, AED 10,000 or more for the full face |
| Hyaluronic acid fillers | Replace lost volume directly | Deep folds and hollows | A day or two of swelling | AED 1,500 to 3,000 a syringe |
Fotona 4D works on skin quality and the mouth area, while HIFU reaches the deeper layer supporting the jawline, so one does not replace the other. Endolift works under the skin, usually in one session; see our Endolift guide. For every option ranked by how much laxity it handles, read how to tighten loose skin in Dubai.
Is Fotona 4D worth it?
It can be, if your skin is only starting to loosen and you want gradual improvement without needles; it is poor value if you expect it to lift sagging jowls or replace filler. The evidence supports a small, real and temporary effect on skin quality, pores and lines around the mouth and eyes. It suits early laxity or tired texture, with a budget for maintenance. People with marked jowls, neck bands or deep folds from lost volume usually get more from HIFU, filler or a surgical consultation. A fair test: ask what change the clinic expects three months after the third session, and what it would suggest if that change does not appear.
Who should not have Fotona 4D, and is it safe for darker skin?
Most healthy adults can have it, but darker skin needs conservative settings, and pregnancy, recent isotretinoin, infection and a fresh tan are reasons to wait.
The Nd:YAG steps are relatively pigment-safe, and Fotona's Nd:YAG laser is cleared for hair removal on Fitzpatrick I to VI skin. The risk sits with heat and the Er:YAG peel step. A 2026 review of treatments for Fitzpatrick IV to VI skin advises avoiding ablative lasers on darker skin because of dark or light patches, and recommends lower energy, fewer passes, longer gaps and strict sun protection. Expect a careful doctor to lighten or skip SupErficial and keep PIANO conservative.
- Pregnancy and breastfeeding: elective laser work is usually postponed; see our guide to aesthetic treatments during pregnancy and breastfeeding.
- Isotretinoin: the label advises waiting six months before laser procedures. A 2017 American Society for Dermatologic Surgery consensus found too little evidence to delay non-ablative lasers, but Fotona 4D includes a light ablative peel, so the doctor should decide.
- Cold sores: laser work in and around the mouth can trigger an outbreak, so mention them; some doctors prescribe an antiviral first.
- A fresh tan, sunburn, broken skin or infection in the area: postpone.
- Undiagnosed brown patches: possible melasma needs a diagnosis first, because it often returns or darkens after heat and laser.
Fotona 4D will not replace a facelift, and the most striking before and after photos usually show mild laxity to begin with. If a clinic promises a result equal to surgery or filler, ask what that is based on.
Licensing: what to check in Dubai, Abu Dhabi and the northern emirates
Fotona 4D uses a prescription medical laser, so it belongs in a licensed healthcare facility: licensed by the DHA in mainland Dubai, the DOH in Abu Dhabi and MOHAP in the northern emirates, with Sharjah (SHA) and Dubai Healthcare City (DHCR) under their own regulators. A salon or spa should not offer it.
The DHA's Standards for Non-Surgical Cosmetic Procedures (version 2.1, effective 18 February 2025) allow lasers to be applied only by healthcare professionals following the treating doctor's written plan, and bar laser technicians from working beyond hair reduction, so a laser hair technician should not run your session. In Abu Dhabi, the DOH licenses facilities and practitioners, checkable through its online services and TAMM. Check the clinic's licence on the right registry, then ask:
- Which Fotona machine will you use: an SP Dynamis, a Dynamis Max or a TimeWalker?
- Are all four steps included, or is this a 2D or 3D session?
- Which doctor sets my plan, and who holds the handpiece?
- What is the facility's DHA, DOH or MOHAP licence number?
- What will you change for my skin type, and will you lighten the peel step?
- If you call it 5D, what exactly is the fifth step?
